What to Bring to the Hospital

All patients are required to go through the registration process. In order for our Patient Service Representatives to perform the registration process as quickly and accurately as possible, please bring with you:

• All active health insurance cards or accident billing information (i.e., worker's compensation/liability)
• Picture Identification (i.e., driver's license, passport)
• Social Security Number
• Name of contact person and their phone number
• Co-payment
• Physician orders

• List of medications
• List of allergies
